Taking the Health Care Journey: What You Need to Know

Facing a hospital stay can be frightening and confusing, and the uncertainly of what to expect can add stress at a difficult time. Who do I talk to if I have any questions during my hospital stay? What happens when I go home and still need care? If I am a caregiver, what services are available to us both in the hospital and when we leave? It can be overwhelming and difficult to figure out how who to talk to about your concerns and determine the best way to develop a plan for when you or your loved one are released from the hospital.
Since this is such an important topic for many seniors, Senior Services of Alexandria and Inova Hospital have teamed up to co-sponsor the first Senior Speaker Series event of the 2018-2019 season: “Taking the Health Care Journey: What You Need to Know.” The event will feature experts from Transitional Care Community Partners, an interagency collaborative led by Inova which was created to inform the public about what to expect during a hospital stay and the many options available for older adults upon discharge.
Representatives of Inova, Senior Services of Alexandria and private service providers will talk about the role of the Case Management team, the differences and services provided by skilled nursing and assisted living facilities, at-home health care and caregiver services, the support offered by hospice agencies, and the various community based services available in Alexandria. Attendees will learn what they need to know to have a better hospital experience, and a smooth transition after discharge.
This is a session that you don’t want to miss, providing a unique opportunity to hear from the full range of service providers that support older adults and their families during and after a visit to the hospital.
